Transport Canada will be replacing existing licences and permits with the new Aviation Document Booklet. The new booklet will be phased in the over a period of years. Documents with the greatest potential for international use will be the priority.

The personal information you provide on this form is collected under the authority of the Aeronautics Act. This information is required for the purpose of the issuance of a Canadian Aviation Document and for the maintenance and distribution of information needed for aviation safety. Refusal to submit the required information will result in the refusal to issue the document applied for. The personal information collected is stored in Personal Information Banks DOT PPU 005 (Aviation Licensing Database) and DOT PPU 020 (Civil Aviation Medical Information System), and will be retained for 20 years following the death of the holder. Under the provisions of the Privacy Act and the Access to Information Act, individuals have the right of access to, and protection of, their personal information. Instructions for obtaining your personal information are provided in Info Source, a copy of which is available in major public and academic libraries or on-line at www.infosource.gc.ca.
