Reviewing your New Aviation Document Booklet
When you receive your booklet, please help us by following the steps below:
Step 1:
- Confirm that the envelope was mailed using your correct name and address.
- Check the condition of your booklet. Your Aviation Document Booklet should consist of 24 pages and should not be damaged.
- The booklet is comprised of three sections:
- Licence and Permit (pages 1-12), which includes the personal data page and your current licence and permit status in the form of printed labels,
- Competency Record (pages 13-14), which includes blank competency record tables, and
- Medical Certification (pages 15-24), which includes your current medical certificate and space for future medical certificate renewals.
- Please pay particular attention to the following pages:

Page 1: Check that the photograph and signature presented are the ones you provided in Part A of your application form. Check the data in all the fields marked with roman numerals (II) to (XI).

Page 2: Sign your name in the space provided at the bottom of this page.

Page 3, and page 4 if applicable: Check that your licence and/or permit information is correct. Use your current licence and/or permit documents to verify and compare the information.

Page 13: Transfer any applicable data from your old pilot proficiency check (PPC) card or other competency record to this page, or retain your current PPC card or competency record until your next check.

Page 16: Check that your medical information is correct. Use your current medical certificate to verify and compare the information.
For your information:
- This booklet is the property of the Government of Canada. It must not be altered in any way.
- This booklet may only be used by the bearer in whose name it has been issued. This booklet is not valid unless signed by the bearer on the bottom of page 2.
- Please review the important information on the front and back inside covers and on pages 2 and 15.
- We recommended that you record the booklet number printed at the bottom of page 24 and keep it separate from the booklet for reference purposes, perhaps with your personal log.
