A commercial helicopter pilot operating a Bell 407 helicopter did not hold a valid Foreign Licence Validation Certificate while flying in Canada and, while operating the aircraft in the ADIZ, did not follow the requirements of filing a flight plan. On further investigation it was also ascertained that certain entries in the aircraft journey logbook were missing the name and signature of the person responsible for making the entry. The pilot was sanctioned with a total licence suspension of 70 days and a monetary penalty totalling $2,250.00.

A commercial pilot operating a Cessna 152 was taxiing for departure at an airport and was instructed by air traffic control to hold short of the active runway. The pilot acknowledged the instruction and then proceeded to enter the active runway necessitating ATC to give overshoot instructions to an aircraft on final for conflict avoidance. The pilot was sanctioned with a $525.00 monetary penalty.

A commercial helicopter pilot operating a Robinson R44 helicopter conducted an approach and landing into a park situated in a built-up area without authorization. The individual was sanctioned with a $750.00 monetary penalty.

Ontario March 29, 2012 CAR 602.31(1) $750 monetary penalty

A private pilot operating a Cessna TU-206G aircraft was taxiing to the active runway for departure and was instructed by ATC to hold short. The pilot acknowledged the clearance and then proceeded to enter the active runway which required a two aircraft on final to overshoot and go-around. The pilot was sanctioned with a $750.00 monetary penalty.

A private pilot writing a commercial pilot exam was found cheating on the exam. The individual was fined $1,000.00 and was not allowed to apply for another pilot exam for one year.

A private pilot operating his aircraft on a VFR flight to an airport was cleared to land on the active runway by air traffic control however; he subsequently landed on the parallel taxiway. The pilot was sanctioned with a $500.00 monetary penalty.

A private pilot operating a Piper Supercub on floats attempted to start his aircraft and inadvertently drifted into another aircraft, making contact with his propeller. The pilot subsequently departed in his aircraft without an inspection after an abnormal occurrence had taken place. The pilot was sanctioned with a $700.00 monetary penalty.

A private pilot flying a Cessna 182 landed at an MF aerodrome without making the mandatory arrival calls on the appropriate MF frequency. Further investigation revealed that the aircraft did not conform to airworthiness requirements prior to being flown. The pilot was sanctioned with a $325.00 monetary penalty and a 10 days licence suspension.

A commercial pilot operating a Diamond DA20 aircraft with another individual on board did not commence the flight with sufficient fuel in the tanks to complete the flight successfully. Consequently, the aircraft’s engine stopped operating two miles short of the destination airport and the aircraft had to be force landed in a farmer’s field. The pilot received a 30 days licence suspension.

An individual operating an ultra-light Beaver RX550 aircraft did so without possessing a flight crew permit. During the flight in question the aircraft crashed and was destroyed. The individual was sanctioned with a $1,000.00 monetary penalty.
