A commercial helicopter pilot from a foreign state operating a Bell 407 helicopter in Canada did not file a flight plan in or out of an aerodrome located within the Air Defence Identification Zone (ADIZ). On further investigation it was determined that the pilot was not in possession of a valid Foreign Licence Validation Certificate and had failed to make the proper entries in the aircraft journey logbook by not entering his name, the date of the flight and his signature. The pilot was sanctioned with a monetary penalty totalling $2,250.00 and a licence suspension totalling 70 days.

An aircraft maintenance engineer who was responsible to submit a Service Difficulty Report (SDR) on an aircraft, failed to submit the report within the allocated time after being requested to do so multiple times. The aircraft maintenance engineer was sanctioned with a $525.00 monetary penalty.

A private pilot operating a Socata TB20 aircraft on a VFR flight entered Class “B” airspace without a clearance and subsequently entered cloud and did not maintain visual reference to the ground. The pilot was sanctioned with a monetary penalty totalling $1,225.00.

An individual operating a Denny Kitfox aircraft without a pilot licence or permit with a passenger on-board crashed shortly after take-off.  The aircraft was destroyed and both occupants sustained injuries. The individual was sanctioned with a $5,000.00 monetary penalty.

A commercial pilot flying a Cessna Caravan under controlled VFR was cleared to maintain a block altitude of 14,000 to15,000 feet. After accepting the clearance the aircraft descended as low as 9,700 feet without authorization.  The pilot was sanctioned with a 10 days licence suspension.

A foreign pilot operating a Lancair aircraft on an IFR flight departed an aerodrome in uncontrolled airspace and was given a clearance by air traffic control to enter controlled airspace at a cleared altitude. The pilot acknowledged the clearance and then proceeded to enter controlled airspace at another altitude. The pilot was sanctioned with a $525.00 monetary penalty.

A private pilot operating a Cessna 172 in the circuit was cleared by ATC to conduct a full stop landing due to wake turbulence from a departing aircraft. The pilot acknowledged the clearance and then proceeded to conduct a touch and go. The pilot was sanctioned with a $525.00 monetary penalty.
