A pilot with a recreational pilot permit departed a lake in a Cessna 150 on floats. When airborne, one float impacted the lake and the aircraft came to rest upright but partially submerged. The aircraft was extensively damaged however; the pilot did not sustain any injuries. The investigation that followed revealed that the pilot did not have a valid medical certificate. The individual was sanctioned with a $700.00 monetary penalty.

A commercial pilot operating a Cessna 152 aircraft on a VFR flight entered Class “D” airspace without a clearance and without establishing two-way communication. A passenger aircraft had to be diverted around this aircraft to alleviate a conflict. The pilot was sanctioned with a $200.00 monetary penalty.

The pilot was sanctioned with a total licence suspension of 108 days.

An airline transport pilot operating a commercial aircraft was identified as operating the aircraft under the influence of alcohol. The pilot was sanctioned with a $3,500.00 monetary penalty.

A private pilot operating a Vans RV4 aircraft was taxiing the aircraft for departure and was instructed by ATC to hold short of the active runway and contact tower. The pilot acknowledged the instruction and then proceeded to take the active runway and departed without a take-off clearance. Another aircraft that had been cleared to land on the same runway was instructed to pull up and go-around and turn right to avoid a conflict with the departing aircraft. The pilot of the Vans RV4 aircraft was sanctioned with a 10 days licence suspension for his actions.
