Subject: Casualty/Marine Occurrence Reporting

A new reporting form, "REPORT OF A MARINE OCCURRENCE/HAZARDOUS OCCURRENCE REPORT" has been jointly developed by the Transportation Safety Board (TSB) and the Canadian Coast Guard (CCG). The new form incorporates the requirements of the CTAISB Act, the Canada Shipping Act and the Canada Labour Code Part II.

The new form (TSB 1808 (06-94) ) is bilingual, back-to-back. To order from DSS, the order number is DSS-7540-21-866-7041. The cost is.12¢ per copy or $6.00 per package of 50.

Until such time as supplies are exhausted or when Section 478 (1) (d) of the CSA is amended along with the subsequent amendments to the Shipping Casualties Reporting Regulations, (form # 82-0001 (3-85) ) known as 1.WR/2.AR form, will be accepted by the TSB.

Should an accident/incident occur that requires the submission of a report, the original of TSB form 1808 (06-94) or the original of form 1.WR/2.AR is to be forwarded to:
