This Bulletin has been replaced by Bulletin No. 03/2011.

The new form (TSB 1808 (06-94) ) is bilingual, back to back. Mariners required to report an occurrence are advised that the forms are free and are available through TSB or Ship Safety offices as in the current practise. For mariners and Ship Safety offices requesting a package of 50, mail orders should be addressed to ADMINISTRATIVE SERVICES, Transportation Safety Board of Canada at the address listed below or faxed to FAX # 819-997-2239. Any enquiries regarding acquisition of these forms may be addressed to Chief, Administrative Services, Jean Laporte, tel. 819-994-8004, on working days between 0800-1600. Please note: DSS is not carrying these forms.
